Medical Consent/Temporary Guardianship: I, the parent/legal guardian, do hereby give permission for the above mentioned child to attend and participate in the above mentioned event. I, the undersigned, in my capacity as parent/legal guardian of the child, do hereby designate temporary custody and guardianship to responsible leaders of the church as may be designated from time to time. I further authorize the individuals to grant permission for medical or dental treatment during the event that the parent/guardian is unable to provide such consent. The undersigned shall be liable and agrees to pay for all costs and expenses incurred in connection with such medical and dental services rendered to the child pursuant to this consent. Should it be necessary for the child to return home for any medical reason or otherwise, the undersigned shall assume all transportation and other related costs.
Liability Release: I, the parent/legal guardian, on behalf of themselves and the child, and in consideration of the voluntary nature of the event the child is able to attend, do hereby release and forever discharge the church and its elders, deacons, pastors, directors, officers, employees, members, adherents, and event-related companies and staff against all losses, claims, suits, and demands, or any liabilities whatsoever, arising from injury or death to the child or other persons involved in the above mentioned activity, or any damage to associated property. Furthermore, the undersigned acknowledge personal responsibility to pay for any damage to property caused by the above mentioned child in any way resulting from any use of the facilities.
Publicity Release: I understand that my child may be photographed and I give permission to the church to use such photos for promotion of the camp.

Warning to the student (a supervised "time-in"). 2. Remove student from activity/group (a supervised "time-out"). 3. Consult with parent/guardian. 4. Remove student from camp (non-refundable).
